“Over the past 2 years my father had been going through a difficult time with his mental and physical health. At one point, things stabilized, but then started going into rapid decline, which the case management/nursing team we had in place was not equipped to handle.

After much research and many phone calls, we were directed to Sasson Senior Services through the Common Ground agency. Diane Sasson and her colleague Jeanne hopped right in, getting an overview of the situation, establishing a rapport with my father, our family, and his medical team, and finding solution after solution–for medical care, housing, complementary medicine, legal advice, you name it.

Fast forward six months, and my father is remarkably stable (and I do mean that–everyone remarks on how much he’s progressed). Besides my father’s hard work and resilient strength, I firmly believe that a great deal of his success is owed to Diane and her warm, consistent support. Her knowledge of the community and her network raised my father’s standard of care significantly. Her warmth allowed my dad to open up and feel like he had an ally, not a manager. And her commitment kept it all steady over the ups-and-downs of illness.

I have had the pleasure of working with Diane, so when mom fell and the time approached when she was preparing for discharge from a nursing home, I immediately called Diane to the rescue. It was clear mom wasn’t safe in her home alone, and the burden of modifying the home plus hiring round-the-clock caregivers was out of the question. She needed medicine, oxygen, therapy, and specialized equipment. Most of all, she needed reassurance. Diane not only found a perfect placement for mom but she also arranged and attended planning meetings to facilitate the smoothest transition. She made sure all the needed services, equipment, and supplies were home. She came to both facilities on moving day to ensure that everything transferred as planned—-including and especially mom!
